Pricing and Ordering Instructions for Departmental Safety Eyewear

Ohio State Departmental UNIV/OSUMC Pricing

One pair of safety eyewear includes a frame, side shields, and polycarbonate prescription lenses.

  • $250 Single Vision (pair)
  • $300 Bifocal Vision (per pair; line or no-line options)
    • Additional $75 charge for double-segment bifocals

Departments requiring specialized eyewear services or materials for employees may request a quote by emailing OPT-EWGOrders@osu.edu.

 

Ohio State Departmental UNIV/OSUMC Ordering Instructions

Internal departments must submit orders through Workday using a Non-Catalog Requisition with the supplier: Optometry Clinic.

Please include the following in your requisition:

  • Employee’s full name (the individual receiving eyewear)
  • Accurate quantities and pricing
  • For multiple employees: use one goods line per person

Note: Requisitions for unknown future employees or blanket purchase orders are not accepted at this time.

To ensure proper billing, please provide your employee with the Workday Purchase Order (PO) number before their appointment with the Optometry Clinic. Departments will be billed via the supplier invoice process after the eyewear has been picked up.
