The Ohio State University College of Optometry's health programs and activities comply with applicable federal civil rights laws and do not discriminate, exclude, or treat people differently based on race, color, national origin, age, disability or sex (including gender identity, pregnancy or related conditions, sexual orientation, sex stereotypes, or sex characteristics, including intersex traits). The Ohio State University College of Optometry provides free aids and services to people with disabilities so they can communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, and other formats). We also provide free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages. If you need these services, please tell us at the time you schedule an appointment or tell a member of your care team.

If you believe The Ohio State University College of Optometry has failed to provide these services or discriminated in another way in its health programs and activities based on race, color, national origin, age, disability or sex (including gender identity, pregnancy or related conditions, sexual orientation, sex stereotypes, or sex characteristics including intersex traits), you can file a grievance with the Civil Rights Compliance Office, Patient Experience, the Section 1557 Coordinator, or the ADA Coordinator. You can file a grievance in person, by mail, by phone, by email, electronically, or anonymously. If you need help filing a grievance, or have questions about Section 1557, the following offices and individuals can help you. Any student, faculty, staff, or covered third party subjected to harassment, discrimination, or sexual misconduct, may report the incident to The Ohio State University Civil Rights Compliance Office.
